Who is AskYourTeam?

AskYourTeam is a company that partners with primary health organisations and medical facilities to conduct surveys and give patient-reported insights to these organisations. This is done to improve patient experience.

What is the purpose of the survey?

To learn more about your experience with primary health organisations and medical facilities. For example: What went well? What didn’t go well? How did this affect your general health and wellness? By sharing your experience, you can help improve and influence patient care.

Can anyone in the family take the survey?

The person who received the treatment should take the survey. However, if that person is under 18, their parent or legal guardian may complete the survey on their behalf, or if English is not the participant's first language, someone else may assist them in entering their feedback.

I have completed a survey previously, should I take it again?

Yes please! The more feedback you can give, the better. But we try not to ask you too frequently and have rules in place so you don't receive too many invitations.

Why did I get the survey invitation when I didn't have the treatment?

You will receive the survey invitation if you are listed as the primary contact for the person who received the treatment.

In this situation, you can choose to:

  • forward it to the person to complete,

  • complete it on their behalf, if they are under 18 and you are their parent or legal guardian, or

  • assist the person with providing feedback if English is not their first language.

Can I print out the survey and send it in by post?

No. To keep the responses anonymous, these surveys can only be completed online.

How do I opt out of taking these surveys?

If you no longer want to receive survey invitations, you can select the unsubscribe link in the email invitation or reply STOP to the SMS/TXT invitation.

Data and privacy


How did AskYourTeam get my contact information?

AskYourTeam receives a secure electronic list of appointments for the current day. This list contains only contact details and very basic demographic data.

How much do you know about me?

We don't know anything about you. Once a survey invite is sent to your email or phone, the contact information is immediately removed, and any responses you provide will be completely anonymous.

How do I know my responses are anonymous?

In order to protect your privacy, your primary health organisation or medical facility is conducting this survey through AskYourTeam, a third party.

AskYourTeam removes any identifying data connecting you to your survey responses. Unless you identify yourself in a free text comment, there is no way to link a survey response to you.

What type of information is collected?

We only collect information about your experience with the primary health organisation or medical facility. You'll be asked some multi-choice questions and have an opportunity to provide comments.

We don't collect information about clinical outcomes (measurable impacts on health, function, or quality of life as a result of care).

What will be done with the results?

The anonymous survey results help primary health organisations (PHOs) and medical facilities understand their patient's experience with them.

To support wider service improvements, PHOs or AskYourTeam may choose to share these anonymous survey results with other healthcare providers and facilities. This could include publishing them in the future to allow for a comparison of experiences.

How do I know my personal information is safe?

AskYourTeam undertook an independent data security evaluation to ensure your information is handled safely, securely, and in accordance with standard primary health organisation and medical facility privacy statements.
